on village facilities at a cost not to exceed $45,000.

Authorization to solicit sealed bids for 1999 Parkway Tree Replacements (Trustee
Berman)
RECOMMENDATION: Approve by motion.
SUMMARY: Due to an increase required tree replacements, sealed bids are
sought for program estimated to exceed statutory allowance of$ 10,000.

The Village is pleased to introduce the new 'Business Showcase Program". The purpose of the
program is to highlight and promote local businesses. If a business is celebrating an anniversary of
their BG store,or coordinating a special event,they may be eligible for the Golden Bison Award.
This month,the Village would like to recognize the Highland Park Hospital Health and Fitness
Center for their efforts in coordinating their first annual Celebrating Fitness 5K on Sunday,
September 26, 1999. The proceeds of the run/walk will benefit the Walter Payton Foundation. The
foundation's primary goal is the funding and support of children's educational programs, and
programs assisting abused or neglected children.
The HPHHFC is located at 1501 Busch Parkway, and began operation in Buffalo Grove in 1997.
Congratulations to Leslie Galvan,Director of Programs and Corporate Services, and Robyn Kove,
Director of Marketing and Member Services, and the other representatives of the Fitness Center!

October is Domestic Violence Awareness Month. The Village of Buffalo Grove believes in a future
free from family violence. To show its support for programs dedicated toward the prevention of
domestic violence,the Village will participate in National Work to End Domestic Violence day on
October 1, 1999. Programs will continue throughout the month of October.
Village of Buffalo Grove Police and Village cars will display bumper stickers indicating domestic
violence will not be tolerated in this community. Village employees will be encouraged to wear
purple ribbons indicating their support of a violence free community. Domestic Violence
information will be posted in all Village buildings and public areas to increase community
awareness.
In addition,local businesses will be encouraged to follow this example and participate in similar
programs.

DECLARING THE VILLAGE OF BUFFALO GROVE'S PARTICIPATION IN
DOMESTIC VIOLENCE AWARENESS MONTH
WHEREAS, The Village of Buffalo Grove believes in a future free from family
violence, and
WHEREAS, October is Domestic Violence Awareness Month, and
WHEREAS, The Village of Buffalo Grove supports programs dedicated toward the
prevention of domestic violence especially the following:
• Village of Buffalo Grove Police and Village cars will display bumper stickers
indicating domestic violence will not be tolerated in this community;
• Village employees will be encouraged to wear purple ribbons indicating their
support of a violence free community;
• Domestic violence information will be posted in all Village buildings and
public areas to increase community awareness, and
• Local businesses owners will be encouraged to participate in similar Domestic
Violence Programs,

After due notice as required by law, a copy
of said publication notice being attached
hereto as Exhibit "C" the Zoning Board of
Appeals held a Public Hearing regarding the
proposed variance on Tuesday, August 17,
1999 at the Village Hall, 50 Raupp Blvd.
The applicant' s representative, Ms .
Natialie Grindle, Acme Wiley Sign
Corporation testified at the hearing and
presented documentary evidence .
No objectors appeared at the hearing.
No written objections were received.
Attached hereto and made a part hereof as
Exhibit "J" are the minutes of the Zoning
Board of Appeals Meeting held on Tuesday,
August 17, 1999 .
IX. FINDINGS :
The Zoning Board of Appeals finds that the
criteria as set forth in Section 14 . 44 . 010,
Subsection A of the Sign Code have been
met in that the Petitioner has shown:
A. 1 . That not allowing the variance will cause
undue and unnecessary hardship due to the
following unique conditions :
a. Walgreens signs are nationally recognized
and needed for identification at a distance .
2 . That granting the requested variance would
not be detrimental to the property owners in
the vicinity.
3 . That the unusual conditions applying to the
specific property do not apply generally to
other properties in the Village, and
4 . That granting of the variance will not
be contrary to the general objective of
this Title of moderating the size, number
and obtrusive placement of signs and the
reduction of clutter.
X. CONDITIONS :
The variance shall be subject to the following
conditions :
Signs are to be as presented per the exhibits
submitted with the application.
Base of sign is to be landscaped.
XI . CONCLUSION:
The Zoning Board of Appeals, by a vote of 4 to 1
recommends to the Village Board, that the request
made by Walgreens, 15 N. Buffalo Grove Road for
variance of B-5 Zoning Ordinance, Section
17 . 44 . 060 .E . 6, pertaining to Signs be GRANTED for
the purpose of installing a Walgreens ground sign
at 15 N. Buffalo Grove Road at the Town Center,
per the drawing submitted, with the base landscaped.

CALL TO ORDER
In the absence of Ch. Richard Heinrich, the Zoning Board of Appeals
meeting was called to order and chaired by Com. Jay Paul at 7:39 P.M.
on Tuesday, August 17, 1999 in the Council Chambers of the Village
Hall, 50 Raupp Boulevard.
ROLL CALL
Commissioners Present: J. Paul, L. Windecker, J.Horwitz,

APPROVAL OF MINUTES
July 20, 1999 - Com. Windecker made a motion to Table. Com. Horwitz
seconded the motion.
Voice Vote - AYE Unanimously. Minutes of July 20, 1999 were tabled.
OLD BUSINESS
A. 15 N. Buffalo Grove Road, Walgreens B-5 Zoning District
Section 17.44.060.E.6. Ground Sign at Town Center
The Walgreens ground sign was tabled on July 20, 1999 because the
proposed sign was not acceptable. The petitioner submitted a new design
for consideration.
Com. Windecker made a motion to remove from Table. Com. Horwitz
seconded the motion. Voice Vote - AYE Unanimously.
Ms. Natalie Grindle, 2480 Greenleaf Avenue, Elk Grove Village, IL 60007
(847-364-350) was sworn in. Walgreens was not represented. Ms. Grindle
requested a variance that would permit a rectangular three (3) foot by
ten (10) foot Walgreens sign with Open 24 Hours and Drive-Thru Pharmacy
on a two (2) foot brick base.
Com. Windecker had no objection to the sign. He said it complied with
limitation he proposed that the height of the sign not exceed twelve
feet.
Com. Krakauer had no objections.
Com. Horwitz said this sign is the same design that was submitted in
July with the exception of the low brick base. He had asked that the
sign be redesigned so that it matched the architecture of the building.
There is no metal on the building, but the sign is framed with metal.
He has seen other foundation signs that tie in with buildings. The
brick base of this sign will be covered with landscaping and only the
box will be seen.
Ms. Grindle explained that Walgreens has a nationally recognized look
that they want to maintain. They did not work with an architect, but
took their basic sign and lowered it as the Zoning Board had directed.
Brick is more expensive and looks better than the clad-ding that was
discussed at the last meeting. The metal supports lights and electri-
cal equipment. It is designed so that it can be easily repaired.
There are water drainage holes that help prevent rust. Bulbs have to
be changed, etc.
Com. Teplinsky said this design complies with the Zoning Board' s
direction. The height meets the twelve (12) foot requirement and the
Walgreens logo can be recognized.
Com. Horwitz repeated his objections to the design of the sign and said
the Walgreens building signs (which have been installed) identify the
store very clearly.
Ch. Paul said appearance is subjective. The ground sign is in good
taste and meets the objectives set forth by the Zoning Board. It will
be seen from a distance coming down Lake Cook Road. He asked how many
votes are now needed to pass a variance?
Mr.Skelton said the revised sign code was passed by the Village Board
on Monday, August 16, 1999. A nine (9) member Zoning Board requires a
majority of a quorum to pass a motion. A quorum is five (5) members so
an affirmative vote of three is needed.
Com. Windecker made the following motion:
I move we recommend to the Village Board of Trustees that the
variance of the B-5 Zoning Ordinance, Section 17.44.060.E.6.
pertaining to Town Center Signs, be granted that would permit
Walgreens, 15 N. Buffalo Grove Road, to install a three (3) foot
by ten (10) foot sign on a two (2) foot brick base at the top of
the berm surrounded by landscaping.
Sign is to conform with the code in accordance with the plans
submitted to and approved by the Building Department in accord-
ance Sign Code, Section 14.44.010, Sub-section A.
Com. Teplinsky seconded the motion.

AN ORDINANCE AMENDING CHAPTER 13.12 OF THE
VILLAGE OF BUFFALO GROVE MUNICIPAL CODE
CONCERNING STORMWATER MANAGEMENT
WHEREAS, the Village of Buffalo Grove has previously established stormwater
management regulations which have proven to result in the construction of effective and
high quality stormwater management systems, and
WHEREAS, the Lake County Board adopted a Watershed Development
Ordinance on June 8, 1992, which was amended on August 10, 1999.
WHEREAS, it is necessary for the Village of Buffalo Grove to amend its
stormwater management regulations to enable the Village Engineer to effectively provide
overall coordination of stormwater management facility design without the inefficiency
and confusion which would otherwise be expected to result if the staff of the Lake
County Stormwater Management Commission were to independently administer Lake
County's Watershed Development Ordinance while the Village staff is simultaneously
administering Buffalo Grove's Stormwater Management regulations.
NOW, THEREFORE BE IT ORDAINED by the Village President and Board of
Trustees of the Village of Buffalo Grove, Lake and Cook Counties, Illinois, as follows:
Section 1: Chapter 13.12.115 of the Buffalo Grove Municipal Code is amended to
read as follows:
Adoption of Lake County Watershed Development Ordinance by Reference: The
provisions of the Lake County Watershed Development Ordinance as approved by the
Lake County Board on August 10, 1999 are adopted herein by reference. In case of
conflict between the provisions of the Lake County Watershed Development Ordinance
and any other provisions of the Buffalo Grove Municipal Code, the most stringent
provisions shall apply. The Village Engineer shall serve as the "Enforcement Officer"
for the purpose of administering the provisions of the Lake County Stormwater
Management Ordinance.
Section 2: The validity of any section, clause, sentence, or provision of this
Ordinance shall not affect the validity of any other part of this Ordinance which can be
given effect without such invalid part or parts.
Section 3: This Ordinance shall take effect and be in full force from and after its
passage, signing, approval, and publication in pamphlet form as provided by law.

The Village Board approved Ordinance 99-62 on August 2, 1999. The approval was conditioned on
the presentation of the revised building elevations to the Plan Commission.
The Plan Commission reviewed the building elevations on August 18, 1999 and voted 7 to 0 to
recommend approval.Please note that various design elements such as the transom windows and
the gabled skylight have been added back to the elevations. Some of these items were proposed for
elimination when the project was presented to the Village Board on August 2.
The revised elevations are attached as Exhibit F dated August 17, 1999 to the draft ordinance.

WHEREAS, the Buffalo Grove Plan Commission has reviewed the
proposed plans and exhibits submitted by the Owner and the request for
approval of said amendment of the Preliminary Plan approved by Ordinance 94-

Preliminary Plan (EXHIBIT C) and the building materials and colors (EXHIBIT F),

careful consideration have concluded that the development of the Property on
the terms and conditions herein set forth would enable the Village to control

Section 1. This Ordinance is made pursuant to and in accordance with the
Village's Zoning Ordinance, Development Ordinance, and the Village's Home Rule
powers. The preceding whereas clauses are hereby made a part of this Ordinance.
Section 2. The Corporate Authorities hereby adopt a proper, valid and
binding ordinance approving the amended Preliminary Plan (EXHIBIT C) attached
hereto, subject to the following conditions:
A. The outdoor playground shall be managed to ensure that no noise
or other nuisances affect adjacent residential areas. Any outdoor activities
other than the playground and activities related to the child care operation
are prohibited unless approved as a special use.
B. Outdoor lighting on the Property shall be managed to prevent glare
and spillover for adjacent properties, and said lighting shall be turned off
no later than 10:00 P.M. each day except for lighting needed for security
and safety purposes as approved by the Village.
C. The "future aquatic center" and future parking ("25 expansion
parking stalls") depicted on EXHIBIT C are subject to approval by the
Village. The Village shall determine if said improvements require any
review and recommendation other than by the Village President and
Village Board.
D. The outdoor lighting plan as set forth in EXHIBITS H and I attached
hereto shall be modified as necessary to have a reading of "zero foot-
candles" at the south property line adjoining residential properties.
Said modifications are subject to Village approval.
E. Owner shall comply with applicable procedures and regulations of
the Village Development Ordinance, including submittal of a final plat and
execution of a Development Improvement Agreement.
Section 3. This Ordinance shall be in full force and effect on
and after its passage and approval. This Ordinance shall not be codified.

A RESOLUTION APPROVING AN AGREEMENT FOR CONSTRUCTION ENGINEERING SERVICES
RELATED TO THE DEERFIELD PARKWAY AND COMMERCE COURT INTERSECTION
IMPROVEMENT
WHEREAS,the Village of Buffalo Grove is a Home Rule Unit pursuant to the Illinois Constitution of
1970; and,
WHEREAS,the Village desires to implement the Deerfield Parkway and Commerce Court
Intersection Improvement; and,
WHEREAS, an agreement for the provision of necessary construction engineering services in a form
which conforms to IDOT funding procedural requirements is needed in order to complete the project;
NOW,THEREFORE,BE IT RESOLVED BY THE PRESIDENT AND BOARD OF TRUSTEES OF
THE VILLAGE OF BUFFALO GROVE,LAKE AND COOK COUNTIES,ILLINOIS as follows:
Section 1: The attached agreement entitled"Preliminary/Construction Engineering Services

As part of the Capital Improvement Program for 1999, staff is requesting authorization to solicit

NOTICE TO BIDDERS
The Village of Buffalo Grove will be accepting bids for Flat Roof Repair and Resurfacing on Village
Campus Facilities.
Bids will be received on or before,but not later than 11:00 A.M. on September 28, 1999 at the Office of
the Village Clerk, 50 Raupp Blvd. Buffalo Grove, IL. 60089, and immediately thereafter all bids will be
publicly opened and read aloud.
Sealed envelopes or packages containing proposals shall be marked"Sealed bids Flat Roof Repair and
Resurfacing".
Detailed specifications are available for bid purposes from the Department of Public Works, 51, Raupp
Blvd. Buffalo Grove, IL 60089.
The Village reserves the right to reject any proposal for failure to comply with all the requirements of this
notice or any of the bid specification and bid summary documents;however it may waive any minor
defects or informalities at its discretion. The Village further reserves the right to reject any and all
proposals or to award a contract which, in its judgement, is in the best interest of the Village.
INSTRUCTIONS TO CONTRACTORS
A. GENERAL: Proposals to be entitled for consideration must be made in accordance with the
following instructions.
B. PROPOSAL FORMS:Proposals shall be submitted in duplicate on forms identical to those
provided. The wording of the proposal form shall not be changed or altered. Enclose in a sealed
envelope bearing the Contractor's name and clearly mark it"FLAT ROOF REPAIR AND
RESURFACING". Address envelope to the Village Clerk. Changes and erasures on the
proposal form must be initialed by the officer signing same.
C. METHOD OF BID:
1. Proposal shall be made on a stipulated lump sum basis for work on which a Contractor
desires to submit a quote.
2. Prices quoted shall be guarantied for a period of sixty(60) days after proposal due date.
D. CONTRACTOR'S RESPONSIBILITY FOR CONDITIONS OF WORK AND SITE:
1. Before submitting a proposal, Contractors shall carefully examine all data made available
in the Contract Documents, fully inform themselves as to all existing conditions and
limitations,the obstacles which may be encountered, and all other relevant matters
concerning the work to be performed, and shall include in the proposal a sum to cover the
cost of all such items to be included in the Contract.
2. No Contractor to whom a Contract is awarded shall be allowed any extra compensation
on account of any matter or thing concerning which such Contractor might have informed
himself prior to the bidding.
E. INTERPRETATION OF CONTRACT DOCUMENTS: If any person contemplating
submitting a proposal for the proposed Contact is in doubt as to the true meaning of any part of
the Contract Documents or other data, submit to the Village a written request for an
interpretation. The person submitting a written request will be responsible for this prompt
delivery. Any interpretation of the Documents will be made only by an addendum duly issued.
A copy of such addendum will be mailed or delivered to each Contractor of record.

SPECIFICATION
GENERAL CONDITIONS
1.0 Purpose and Scope
A. Purpose: The purpose of the project is to provide for scheduled maintenance and repair
of the flat roofs.
B. Scope: The contractor shall complete all work as specified. The work in general is
described as Repair and Resurface Flat Roof System.
1.01 Supervision - This contract will be under the guidance of the "Superintendent"of
Public Works Operations or his authorized representative. Any alterations or
modifications of the work performed under this contract shall be made only by written
agreement between the Contractor and the Superintendent and shall be made prior to
commencement of the altered or modified work. No claims for any extra work or
materials shall be allowed unless covered by written agreement.
1.02 Work Sequence and Timing
A. Contract Time- The work will be substantially completed within 30 days from the date
the Contract is awarded and completed and ready for final payment within 45 days from
the date when the contract was awarded.
B. Working Hours -The Contractor will be allowed to schedule his normal work hours
between the hours of 7:00 A.M. and 4:00 P.M. Monday through Friday. Work during
other hours will be allowed only as authorized by the Superintendent.
C. Concurrent Operations- This contract is a non-exclusive contract with the Village.
The Village reserves the right to use other Contractors or its own employees to perform
work similar to that being performed under the terms of this contract. Performance of
work by others shall be construed as being consistent with the terms of this contract and
shall not be cause for the Contractor to cease performance of work as directed.

1.03 Insurance- The Contractor shall procure and maintain for the duration of the contract,
insurance against claims for injuries to persons or damages to property which may arise
from or in connection with the performance of the work hereunder by the contractor,his
agents, representatives, employees, or subcontractors. The cost of such insurance shall be
included in the Contractor's bid.
A. Minimum Scope of Insurance
Coverage shall be at least as broad as:
1. Insurance Services Office Commercial General Liability Coverage
("occurrence"form CG0001, Ed.11/85)/and/or Village and Contractors
Protective Liability policy with the municipality stated as named insured: and
2. Insurance Services Office form number CA 0001 (Ed.1/87)covering Automobile
Liability, code 1 "any auto"' and endorsement CA 0019 (Ed. 12/88) changes
in Business Auto and Truckers coverage forms—insured contract: and
3. Workers' Compensation as required by the Labor Code of the State of Illinois
and Employers Liability insurance.
B. Minimum Limits of Insurance
Contractor shall maintain limits no less than:
1. Comprehensive General Liability: $1,000,000 combined single limit per
occurrence for bodily injury,personal injury and property damage. The general
aggregate shall be twice the required occurrence limit. Minimum General
Aggregate shall be no less than$2,000,000.
2. Automobile Liability: $1,000,000 combined single limit per accident for bodily
injury and property damage.
3. Workers' Compensation and Employers' Liability: Workers' Compensation
limits as required by the Labor Code of the State of Illinois and Employers'
Liability limits of$1,000,000 per accident.
4. Builder's Risk: Shall insure against"All Risk"of physical damage,including
water damage (flood and hydrostatic pressure not excluded),on a completed
value basis.
C. Deductibles and Self-Insured Retentions
Any deductibles or self-insured retentions must be declared to and approved by the
Village. At the option of the Village,the insurer shall reduce or eliminate such
deductibles or self-insured retentions as respects the Village, its officials and employees;
or the Contractor shall procure a bond guaranteeing payment of losses and related
investigation, claim administration, and defense expenses.

D. Other Insurance Provisions
The policies are to contain, or be endorsed to contain,the following provisions:
1. General Liability and Automobile Liability Coverages
a. The Village, its officials, employees and volunteers are to be
covered as insureds as respects: liability arising out of activities
performed by or on behalf of the Contractor;products and completed
operations of the Contractor;premises owned, leased or used by the
Contractor; or automobiles owned, leased,hired or borrowed by the
Contractor.
The coverage shall contain no special limitations on the scope of
protection afforded to the Village, its officials, employees, or
volunteers.
b. The Contractor's insurance coverage shall be primary insurance
as respects the Village, its officials, employees, and volunteers.
Any insurance or self-insurance maintained by the Village, its
officials, employees, or volunteers shall be excess of Contractor's
insurance and shall not contribute with it.
C. Any failure to comply with reporting provisions of the
policies shall not affect coverage provided to the Village, its
officials, employees, or volunteers.
d. Coverage shall state that Contractor's insurance shall apply
separately to each insured against whom claim is made or suit is
brought,except with respect to the limits of the insurer's liability.
2. Workers' Compensation and Employers' Liability Coverages
The insurer shall agree to waive all rights of subrogation against the Village, its
officials, employees, and volunteers for losses arising from work performed by
Contractor for the Village.
3. All Coverages
Each insurance policy required by this clause shall be endorsed to state that
coverage shall not be suspended,voided, canceled, reduced in coverage or in
limits except after thirty (30)days prior written notice has been given to the
Village but shall remain in effect throughout the life of the project.

E. Acceptability of Insurance
Insurance is to be placed with insurers with a Best's rating of no less than B+.
F. Verification of Coverage
Contractor shall furnish the Village with certificates of insurance naming the
municipality as an additional insured, and with original endorsement affecting
coverage required by this clause. The certificates and endorsements for each insurance
policy are to be signed by a person authorized by that insurer to bind coverage on its
behalf. The certificates and endorsements may be on forms provided by the Village
and are to be received and approved by the Village before the work commences. The
The Village reserves the right to request full certified copies of the insurance policies.
G. Subcontractors
Contractor shall include all subcontractors as insureds under its policies or shall
furnish separate certificates and endorsements for each subcontractor. All coverages
for subcontractors shall be subject to all of the requirements stated herein.
H. The Village of Buffalo Grove,its officers,employees, and agents shall be named as
additional insured on all insurance policies.
1. Indemnity Hold Harmless Provision
To the fullest extent permitted by law,the Contractor hereby agrees to defend,
indemnify and hold harmless the Village, its officials, agents, and employees
against all injuries, deaths, loss, damages,claims,patent claims, suits, liabilities,
judgements, cost and expenses,which may in any way accrue against the Village,
its officials, agents, and employees, arising in whole or in part or in consequence of
the performance of this work by the contractor, its employees, or subcontractors,
or which may in any way result therefore, except that arising out of the sole legal
cause of the Village, its agents or employees. The Contractor shall,at its own expense,
appear, defend and pay all charges of attorneys and all costs and other expenses
arising therefore or incurred in connections therewith, and, if any judgement shall be
rendered against the municipality, its officials, agents and employees, in any such action,
the Contractor shall, at its own expense, satisfy and discharge the same. Contractor
expressly understands and agrees that any performance bond or insurance policies
required by this contract, or otherwise provided by the Contractor, shall in no way limit
the responsibility to indemnify,keep and save harmless and defend the Village, its
officials, agents and employees as herein provided.
1.04 Bid Bond-A bid bond, cashiers check or certified check in the amount of 10%of
the total base bid shall accompany the proposal. Checks must be payable to the
Village of Buffalo Grove.

1.05 Performance Bond—A Performance Bond shall be furnished in the amount of 100%of
the bid. Performance bond of successful bidder shall be conditioned on the faithful
performance of the requirements of the contract, and shall have as surety a corporate
surety authorized to act as such in Illinois and that the Contractor will be responsible for
all claims for injuries to persons or damages to property or premises arising out of or in
connection with his operations prior to the acceptance of the finished work or supplies,
and shall guarantee to indemnify and save the Village and its officers and employees
harmless from all costs, damages and expenses arising out of or by reason of the
contractor's failure to comply and perform the work and complete the contract in
accordance with the specifications.
1.06 Basis of Award- The Village reserves the right at its sole discretion to award a contract
which is in the best interest of the Village and which provides the needed services within
the time frame identified.
1.07 Notice to Proceed- The Contractor and subcontractors shall not commence work under
this contract until the insurance has been approved and the Village has issued a purchase
order.
1.08 Notice-All notices required by the contract shall be given in writing.
1.09. Nonassignability- The Contractor shall not assign the contract or any part thereof,to
any other person, firm or corporation without the previous written consent of the Village.
Such assignment shall not relieve the Contractor from his obligations, or change the
terms of the contract.
1.10 Default- In the event of the Contractor's nonperformance or breach of the contract
terms,the contract may be canceled, in whole or in part,upon the Village's written notice
of default and the Contractors failure to resolve such default within ten(10) days of the
notice. The Contractor shall be liable to the Village for all costs incurred in completion of
the contract.
1.11 Delay- The Contractor shall not be liable in damages for delay in performance when
such delay or failure is the result of fire, flood, strike,the transporting carrier, act of God,
or by any other circumstances which are beyond the control of the Contractor. Under
such circumstances,however,the Village may, at its option, cancel the contract.
1.12 Non-Waiver Of Rights-No delay or failure by either parry to enforce any of the
provisions of the contract or to exercise any right therein granted shall be deemed a
waiver thereof or limitation in any respect on the right of either parry then and thereafter
to enforce all provisions of this contract and to exercise any such right.
1.13 Contractor Certifications-A bidder shall execute the Contractor Certifications and
submit along with bid.

1.14 Contractors Responsibility
A. The Contractor shall provide equipment,materials, labor and tools to
complete repairs, removal and installation as required.
B. The Contractor is solely responsible for construction means,methods,
techniques, sequences and procedures and for safety precautions and
programs.
C. All equipment and installation materials shall be delivered to the
installation contractor with freight and all handling charges included
in the proposed price. No materials may be shipped directly to the
job site without the prior approval of the Village.
D. The Contractor shall minimize disruptions to the Village's normal
operations and shall render all reasonable assistance when required.
The Contractor shall remove all materials, debris and rubbish from
the site on a daily basis.
E. The Contractor is responsible for acts and omissions of his employees
and his sub-contractors and their agents.
F. The Contractor shall protect the Village's property and all persons and
shall be liable for any and all damages resulting from his operations.
G. The Contractor shall be responsible for the repair or replacement of any
facility or utility damaged during a repair, replacement or installation.
1.15 Employment Preference- The Illinois Human Rights Act applies fully to this contract
and this contract shall be performed in all respects in compliance with the Illinois Human
Rights Act 775 ILCS 511-101, et seq.,and the Illinois Public Works Employment
Discrimination Act 775 ILCS 10/0.01, et seq.
1.16 Prevailing Wage- The State of Illinois requires under Public Works contracts that the
general prevailing rate of wages in this locality be paid for each craft or type of worker or
mechanic needed to execute the work hereunder. It shall be mandatory upon the
Contractor to whom the contract is awarded and upon any subcontractor under the
Contractor. This requirement is in accordance with Illinois Compiled Statues 130/0.01,
et. seq.
1.17 Method of Payment- Payment will be made on a lump sum,based on work completed
and approved. The Village will pay according to the Illinois Prompt Payment Act.
1.18 Warranty-All material and workmanship shall be guaranteed according to
manufacturer recommendation after inspection and approved by the Superintendent or his
designated representative. Any defects found shall be corrected in a timely manner at the
Contractors expense.

Re: AUTHORIZATION TO BID FLAT ROOF MAINTENANCE
Bill, as part of the Capital Improvement Program for 1999, staff presented a request to
complete the 5 year preventive maintenance roof coating and repairs to the flat roofs on all
village owned facilities with membrane roofs.
The warranty on our membrane roof system recommends that a fibrated aluminum roof
coating be applied to the flat roof surface every 5 years.
We respectfully request authorization to solicit sealed bids for the flat roof repairs and
resurfacing.

NOTICE TO BIDDERS
The Village of Buffalo Grove is accepting sealed bids for the PARKWAY TREE
REPLACEMENT PROGRAM. Bids will be received on or before, but not later than 10:00
A.M. on October 7, 1999 at the Office of the Village Clerk, 50 Raupp Blvd.,
Buffalo Grove , 1160089 and immediately thereafter all bids will be publicly opened and read.
Sealed envelopes or packages containing proposals will be marked ABID FOR PARKWAY
TREE REPLACEMENT=
Detailed specifications are available for bid purposes from the Department of Public Works, 51
Raupp Blvd., Buffalo Grove, Il 60089.
The Village reserves the right to reject any proposal for failure to comply with all the
requirements of this notice or any of the specifications and summary documents; however, it
may
waive any minor defects or informalities at its discretion. The Village further reserves the right
to reject any and all proposals or to award a contract which in its judgement, is in the best
interest
of the Village.

REQUEST FOR PROPOSAL
PARKWAY TREE REPLACEMENT
GENERAL
The following request for proposal involves providing two hundred (200) shade trees to the
Village of Buffalo Grove, Illinois, as herein specified and directed by the Superintendent of
Public Works Operations, Department of Public Works, 51 Raupp Boulevard, Buffalo Grove,
Illinois, 60089.
Only stock grown in hardiness zone 5 or other northern zones with comparable climatic
conditions as that of the Village of Buffalo Grove will be acceptable.
All proposals will include the growing location, state and county of all trees offered in each lot
bid.
All trees purchased under this contract will be scheduled for planting in the fall of 1999. Should
weather or other factors prevent completion of the program this fall, the Village reserves the
right to order the remainder of the trees next spring.
All trees shall be balled and burlapped. All trees shall be a minimum 2-1/2" caliper measured 6"
above planting level. All trees shall have straight trunks and good branching. Branches shall
begin from five (5)to six (6)feet above ground level and shall be true to name and no
substitution in species will be accepted unless authorized by the Superintendent of Public works
Operations or his designated representative.
A Village employee will visit the growing site and mark all trees for planting by means of a
permanent tag. Only those trees tagged by the Village will be accepted for planting and
payment. Final acceptance will be done after planting is completed in the Village.
GUARANTEE
Any tree, which in the opinion of the Superintendent of Public Works Operations or his
authorized representative, that is dead, shall be replaced within thirty (30) days after formal
notification from the Village of Buffalo Grove.
All trees shall be fully guaranteed for a period of one (1)year from the date of delivery. A
written guarantee shall be attached to the bid. Said guarantee to stipulate the nursery locations
and types of trees covered by the guarantee.

REJECTION OF MATERIAL
The Superintendent of Public Works Operations or his authorized representative, reserves the
right to reject any plant material which does not comply with the specifications, and all rejected
plant materials shall be immediately removed by the contractor from the premises of the Village
and the Superintendent of Public Works Operations will not assume any responsibility for such
rejected material.
QUANTITY
The right is reserved to increase or decrease the quantities shown depending upon the
availability of stock and inspection results.
DELIVERY
The contractor shall consult with the Superintendent of Public Works Operations or his
authorized representative, concerning the details and scheduling of the work contemplated
herein, and shall be governed by the decisions of said Superintendent.
No material shall be dug or loaded for shipment from the nursery when the temperature is below
thirty-two degrees (32 degrees) above zero (Fahrenheit), and when the depth of frost in the
grounds exceeds one inch(1").
Roots of material shall not be subject to temperatures below thirty-two degrees (32 degrees)
above zero (Fahrenheit). All material whose roots have been frozen shall be rejected.
The contractor shall comply with all good trade practices to insure arrival of stock at the stated
delivery point in the best condition for successful growth without injury of any nature.
All trees shall be delivered F.O.B. to the Department of Public Works, 51 Raupp Blvd., Buffalo
Grove, Illinois or as directed by the Superintendent of Public Works Operations.
SUBSTITUTIONS
Quotations on this material must comply with all provisions of the above specifications. Any
variation from these specifications must be noted on page 12 and page 16 describing the
quantity, specie, unit cost, and the total cost.

ADOPTED STANDARDS
All plants shall conform in size and grade in accordance with the "Horticulture Standards" most
recently adopted by the American Association of Nurserymen for deciduous shade trees.
All plants will be handled in accordance with accepted horticultural practices and professional
standards as recommended by the Illinois Landscape Contractors Association.
INSPECTION OF PLANT MATERIAL
The nurseries supplying plant material may be visited before award of the contract. Material,
which is suitable for the Village's requirements, will be inspected and selected by the
Superintendent of Public Works Operations or his authorized representative.
Approval of material on such an inspection shall not be construed as an acceptance of material.
Final acceptance will be made when the material has been delivered and inspected by the
Superintendent of Public Works Operations or his authorized representative at the site of
delivery.
QUALITY OF PLANT MATERIAL
All material shall meet the requirements of the State and Federal laws with respect to disease and
insect infestation.
Material shall be in a live, healthy condition free from dead branches, blemishes, scars decayed
spots, frost cracks, disfiguring knots, bruises, broken bark or mutilation of any nature.
Specified varieties of any species shall be specimen type material and shall be first class
representatives of their normal species and varieties. They shall have well-developed root and
branch system, reasonably straight stems, and a well-defined single leader.
Material shall be nursery grown with a good compact, fully developed fibrous root system which
has been developed by proper cultural treatment and is sufficient enough to insure plant growth.
Only material dug with the earth ball from the nursery growing location will be acceptable.
Material with a composition or processed ball will be rejected.

OPTION A:
The following request for proposal involves providing two hundred (200) shade trees to the
Village of Buffalo Grove, Illinois, as herein specified to be planted by Village personnel. A one-
(1)year guarantee is required.
OPTION B:
The following contract specifications involve the furnishing and planting of two hundred (200)
shade trees at various locations in the Village of Buffalo Grove, Illinois, as herein specified.
PLANTING
Tree shall be planted in holes at least twelve (12) inches wider than the diameter of the ball
and a minimum of two (2) feet in depth. In unfavorable ground conditions, the contractor shall
place approximately six (6) inches of loose topsoil in the bottom of the excavation before
planting the tree. All excess spoilage and/or soil are to be removed by the contractor. All trees
must be watered immediately after planting by the contractor.
All plantings must be pruned.
Each tree shall be planted plumb and at the same level as where it stood in the nursery in relation
to finished grade. Backfill shall be properly fertilized with organic fertilizer and shall be
thoroughly watered when the hole is two-thirds (2/3)full of topsoil.
After watering, the filling shall be completed and the soil thoroughly tamped. After planting,
four-(4)inch mulch of wood chips or an approved equal shall be applied over the disturbed
ground and a shallow watering basin provided around the tree.
VILLAGE OF BUFFALO GROVE PUBLIC CONTRACTS
The Village of Buffalo Grove is required to represent and obtain certain information in the
administration and awarding of public contracts. Please review the following information, which

should be executed and notarized where necessary. It shall be a requirement in the award of this
contract, and possibly, any future contracts.

have been successful in selecting clean vehicles for a very reasonable price.

Bill, two unmarked vehicles are due for replacement this fiscal year:

A total of $36,886 is reserved for the replacement of these vehicles..
I have discussed the above with Mr. Rigsby, and we would like to proceed with the
replacements in time to put the above vehicles in the October NWMC auction.
Since 1983 we have acquired our under-cover fleet by purchasing either used rental cars
or new, end of the year model vehicles. We have found this to be a relatively low cost
option that has ensured plenty of versatility and utility for our investigative officers.
Because of the mission and deployment of those officers, it is wise that the types of
vehicles they use not reflect typical municipal or police operations. Therefore,
respectfully request you seek approval from the President and Board of Trustees to waive
bids and authorize purchase of two replacement vehicles for the above units with total
costs not to exceed the above sum.
Please note that our options for purchasing used rental vehicles is limited, but I fully intend
to explore that option.

agreement with Andres Medical Billing,Ltd.,to collect fees for services rendered by the Buffalo
Grove Fire Department for non-resident ambulance transportation.

SUBJECT: NON-RESIDENT AMBULANCE TRANSPORTATION FEE
Bill, below is the narrative I provided for the Village Board Agenda on the new agenda
system. After I typed the narrative I found out you could not print the narrative if it was
longer than the box provided, so I have put it in memo format and have attached it to the
agenda system for viewing.
When the initial Non-Resident Ambulance Fee was established by ordinance in 1991 our
early years collection rate was approximately 80%., now it is approximately 50% to 60%.
Than the procedure we used then and still currently use starts with the Fire Department
making a "masked" copy (the copy masks out any patient confidentiality information) of
the Emergency Medical System (EMS) run report that is created by the responding
Paramedics and send the reports to the Finance Department. Than the Finance
Department then creates individual bills and mails them out, which hopefully results in
payment. After additional notices unpaid bills are sent to a collection agency.
More and more over the last several years we are finding that in order to collect from
patients with Medicare and from insurance companies we have to provide the proper
billing codes for our services and in some cases prove-up medical care. This, when we
have the capability, is very time consuming and in most cases we are not capable of
providing this information. Than the bottom line is we never receive reimbursement and
as a result have seen our reimbursement rate falling significantly. Not to mention the staff
time involved answering questions and responding to inquiries.
This global problem has created a new private industry market for medical services billing.
We investigated most of the companies in our area that provide these services and
narrowed them down to two finalists, Andres Medical Billing, Ltd., located in Arlington
Heights and Metro Paramedic Services, Inc., located in Elmhurst. Than the basic system
and costs (10% of collections) were identical with the exception of how we were paid.
Andres Medical Billing would collect the payments and send them to the Village so the
Village would deposit the payments and then send them their billing fee of 10%. Metro
Paramedic Services collects all payments, deposits them in their account, deducts their
10% fee and sends one check to the Village. In discussions with Bill Brimm regarding the
differences, it was felt that we would be more comfortable with the billing method of
Andres Medical Billing. This was confirmed with several Metro client checks we made,
that they felt uncomfortable with the process because it was extremely difficult to track
payments and felt the system was ripe for mistakes or even unscrupulous business
practices.
Finally, we are requesting that we change from the current flat fee of$300 for non-resident
ambulance transportation to a schedule of fees, as outlined in the Ordinance change,
allowing for fees to be charged for services over and above the base rate of $300 for the
ambulance and personnel. Approximately 50% of the fire departments that charge for
ambulance service (almost all fire departments have a ambulance fee) have converted to
this type of billing. A typical bill for an auto accident would include $300 for the
transportation BLS base rate, $56 for mileage (7 miles x $8/mile), $75 for spinal
immobilization and $30 for bandaging for a total of $461. For all Medicare billing we are
considered a Method 1 assignment which basically means we can only collect on the
base rate of$300, which is approved at approximately $150, which is then paid at a 80%
rate which means we collect approximately $120.
On the basis of the aforementioned I request that the Village Board of Trustees modify
the current Non-Resident Ambulance Fee Ordinance allowing for an ambulance billing
schedule of fees and also authorizing the Village Manager to enter into an outside billing
service agreement with Andres Medical Billing, Ltd., 208 W. University Drive, Arlington
Heights, to collect fees for services rendered by the Buffalo Grove Fire Department for
non-resident ambulance transportation.
ORDINANCE NO. 99-
AN ORDINANCE AMENDING CHAPTER 3.36,
NON-RESIDENT AMBULANCE FEE OF
THE VILLAGE OF BUFFALO GROVE MUNICIPAL CODE
WHEREAS, the Village of Buffalo Grove is a Home Rule Unit pursuant to the
Illinois Constitution of 1970; and
NOW THEREFORE, BE IT ORDAINED BY THE PRESIDENT AND BOARD OF
TRUSTEES OF THE VILLAGE OF BUFFALO GROVE, COOK AND LAKE COUNTIES,
ILLINOIS:
SECTION 1. Section 3.36.030 of the Village of Buffalo Grove Municipal Code
is hereby amended to read as follows:
Section 3.36.030 Fee Established.
A. The following fees shall be changed to any non-resident who utilizes the
rescue services of the Buffalo Grove Fire Department:

transportation.
SECTION 2. That this Ordinance shall be in full force and effect from and after
its passage, approval and publication. This ordinance may be published in pamphlet form.

to prohibit parking along the involved street in certain locations.
NOW, THEREFORE, BE IT ORDAINED BY THE PRESIDENT AND THE BOARD OF
TRUSTEES OF THE VILLAGE OF BUFFALO GROVE, COOK AND LAKE COUNTIES,ILLINOIS:
Section 1: Title 10, Chapter BG-11, Section BG-11-1308 of the Village of Buffalo Grove
Municipal Code,is amended by adding the following:
106. On the north curbline of Bernard Drive between the east property line of 594 Bernard
Drive and west property line of 674 Bernard Drive.
Section 2: Any person violating any portion of this Chapter shall be punished
according to the provisions of Chapter 1.08 of the Buffalo Grove Municipal Code.

I am requesting a bid waiver for the implementation of a State of Illinois sponsored and funded
wireless network to connect remote village buildings to the local campus. I am requesting this
project be awarded to RMS Business Systems of Buffalo Grove because they are uniquely qualified
to deliver this project for three reasons.
1) Synergy created by combining projects and ensure compatability between the two projects.
Several members of the Buffalo GroveESDA work for RMS Business Systems. ESDA is in the
process of implementing a similar system to allow mobil-com the ability to link the van to the
villages' computer network to access village computer resources. By combining this wireless
network with the ESDA project we will eliminate duplicate efforts and incompatable systems
between the two projects, save the village money by implementing one larger coordinated system,
and allow remote buildings and some vehicles access to the villages computer system.
2) Depth of knowledge about the Villages network and environment.
RMS designed,installed and configured the core network when first implmented in 1995. They
have provided advanced trouble-shooting and have assisted with developing the evolving vision of
how to best design and maintain our growing network.
3) Depth of Knowledge on the proposed solution.
RMS has designed/installed this type of wireless network other entities. They have been keeping me
informed of the new technology and recently recommended its implementation should the village
begin to get charged for the use of the Institutional Network. They have demonstrated this
technology at trade shows and municipal user conferences and have demonstrated their depth of
knowledge on this topic.
Attached you will find a copy of the request that was made to the state which resulted in the grant
to the Village of Buffalo Grove $74,500.00 to implement this project. In short I am requesting that
RMS Business System of Buffalo Grove be awarded the Wireless Network Project not to exceed the
State of Illinois Grant of$74,500.00.
