A RESOLUTION ACCEPTING THE ''AGREEMENT FOR CONSULTANT SERVICES" FOR THE
DESIGN (PHASE II) FOR THE BUFFALO GROVE ROAD (LAKE-COOK ROAD TO DUNDEE
ROAD) STREET IMPROVEMENT
WHEREAS, the Presjdent and Board of Trustees of the Village of
Buffalo Grove desire to cause the implementation of the Buffalo Grove Road
(Lake-Cook Road to Dundee Road) Street Improvement; and,
WHEREAS, the approval of an agreement with a consulting engineer
is required in order to proceed with the necessary engineering of the project;
NOW, THEREFORE, BE IT RES0LVED by the president and Board of
Trustees of the Village of Buffalo Grove, Cook and Lake Counties, Illinois
that the agreement entitled, "Agreement for Consuitant Services between the
Village of Buffalo Grove and Midwest Consulting Engineers, Inc.,,is hereby
approved and that the Village Manager is hereby authorized and directed to
execute the agreement. A copy of said agreement is attached hereto and made
a copy hereof.

(SP 6.48) Right-of-way documents for 9 parcels
will be prepared. The documents sha1l include
right-of-way parcel plats (showing the total

right-of-way plans. A plat of highways is not
reguired for this project and sha1l not be
provided. The CONSULTANT shall furnish title
lettera.
(SP 6.49) This work is deleted. Staking of

retainage provisions contained in Section 2.42 of
the STANDARD PROVISfONS shal1 not apply to Direct
Non-Salary Costs. The attached Exhibits A, B, and
C are to be used as set forth in Section 2.42 of
the STANDARD PROVISIONS. Invoices shall be
prepared in accordance with Section 2.4I.
.2 The Prime Compensation may be increased or
decreased by subsequent agreement between the
contractual and approvinf parties if there is a
major change in the scope, character or complexity
of the work. prime Compensation and Specific
Rates fo! Extra work may be adjusted in accordance
with Section 2.2L of the STANDARD PROVISIONS, if
the duration of the work in any phase extend.s more
than six months beyond the date of completion
stated in Section 4 of this AGREEIIENT, due to no
fault of the CONSUL?ANT.
.3 In all instances where the VILTAGE sha11 reguest
specific services of the CONSULTANT, the
CONSULTANT prior to conmitment shal1 indlcate to
the VIILAGE rdhether the work shall be considerednextra work'. When requested by the VfLIAGE, the
CONSULTANT sha11 prepare and submit a cost
estimate for such extra work. fn the event the
CONSULTANT, upon written order from the VILLAGE,
is required to perform Extra work, he sha11, inaddition to the prime Compensation, receive
payment for such Extra Work on the following
basis:

Compensation for each item of Extra

the specified hourly rates of pay for
each class of employee as listed in

CONSULTANT directly utilized each such

plus actual expenses for transportatj.on,
subsistence, and electronic computer

Any other expbnses incurred incidental

The scope of the pavement evaluation will consist of the followlng:
a. A reconnaissance and visual survey of the existing pavement
condi ti on.
b. Consultation with Midwest Engineers for review of pavement
surveY and core I ocati ons.
c. Determination of existing pavement cross-section by coring -' of specimens. Ten (10) core locations are-proposed, spaced
at alproximately 600 foot intervals along the alignment.

A soils profile will be prepared, in accordance with applicable specifl--
catlons, showing lhe soil types and test data. }le understand that repro-ducible
copies of the plan and profile showing existing and proposed grade will be
provided by Midwest Consulting Engineers, Inc.
The results of the field exploration and laboratory testing will be utl-
lized in our analysis and formulation of our reconunendat i ons. Recommendations
regarding earthwork, and treatrnent of in-place soils will be presented in a
written report prepared by a registered Professional Engineer.
Fees
lle propose to provide this work at the unit rates quoted on the attached
