ALLOCATION OF OPIOID SETTLEMENT
��V� FUNDS FOR COUNSELING SERVICES AT
BUFFALO GROVE HIGH SCHOOL
WHEREAS the Village of Buffalo Grove is a home rule unit pursuant to the
Illinois Constitution of1970; and
WHEREAS the Village of Buffalo Grove has received funds as part of a
settlement agreement related to the Opioid Crisis (the "Funds"), which are
authorized for use in community mental health and substance abuse initiatives; and
WHEREAS the Village has identified the provision of counseling services
through OMNI Youth Services and District 214 as an appropriate and necessary use
of the Funds to support the students and families of Buffalo Grove High School; and
WHEREAS the Village will allocate an annual amount of$58,000 from the
Funds over the four-year period after the execution of this Agreement, to
OMNI/EDGE to hire a Licensed Professional Counselor (LPC) in-house at Buffalo
Grove High School to provide mental health and substance abuse support tailored to
the needs of the students' and family's needs.
NOW THEREFORE BE IT ORDAINED by the President and Board of Trustees of the
Village of Buffalo Grove, Cook and Lake Counties, Illinois, as follows:
SECTION 1. The foregoing recitals are hereby adopted and incorporated and made a
part of this Ordinance as if fully set forth herein.
SECTION 2. The Village of Buffalo Grove is authorized to enter into a Donation
Agreement (attached as Exhibit A) with OMNI Youth Services and District 214 to
allocate opioid settlement funds for the purpose of providing counseling services at
Buffalo Grove High School.The funds will support services including individual
therapy, group therapy, family counseling, and mental health education for students
and families, subject to final review and approval of the Village Attorney.
SECTION 3. If any section, paragraph, clause or provision of this Ordinance shall be
held invalid, the invalidity thereof shall not affect any other provision of this
Ordinance.
SECTION 4. This Ordinance shall be in full force and effect from and after its passage
and approval and shall be codified.

DONATION AGREEMENT
This Donation Agreement (this "Agreement") is entered into by and between Village of
Buffalo Grove, an Illinois municipal corporation("Donor"),Omni Youth Services("Recipient"),
and District 214 on behalf of Buffalo Grove High School ("Recipient"), as of this 1 I�day of

RECITALS
WHEREAS,Donor has received certain funds as a result of settlement of litigation related
to the Opioid Crisis (the "Funds");
WHEREAS, Recipients provides counseling, support, and resources for the students of
Buffalo Grove High School;
WHEREAS, Donor has agreed, over the four year period after the execution of this
Agreement, to provide an annual amount of$58,000.00 of the Funds to Recipient to be used by
Recipient as part of its mission to provide mental health and substance abuse counseling and
resources for four years. (the Donation");
WHEREAS, Recipients have agreed to utilize the Donation as part of its mission to
provide counseling and support to the students of Buffalo Grove High School.
NOW THEREFORE, in consideration of the foregoing recitals, the parties hereby agree
as follows:
1. Donation. Subject to the limitations outlined in the restrictions on use of the Funds
and this Agreement, Donor hereby agrees to make the Donation to Recipient. It is expressly
understood and agreed that the Donation set forth herein is not a general obligation or debt of the
Village and shall be payable only by the Funds actually received by Donor, if any, during the term
of this Agreement.
2. Use of Donation Recipient agrees to use the Donation only as set forth in this
Agreement and as set forth on E�ibit A which is attached hereto and incorporated herein. Further,
Recipient agrees to cooperate with the Donor in any audit or accounting as a result of the Donation.
3. Counterparts. This Agreement may be executed in counterparts, each of which
shall be an original, but all of which shall constitute one instrument.
4. Successors and Assisns. This Agreement shall not be assigned.
5. Total Agreement. This Agreement (i) contains the entire understanding between
the parties hereto with respect to the transactions contemplated herein; and (ii) may be altered or
amended from time to time only by written instrument executed by both parties hereto.
6. Choice of Law. This Agreement shall be interpreted in accordance with the laws
of the State of Illinois.

7. Term. The term of this Agreement shall be four years after execution.

Exhibit A
l. With $58,000 being provided annually, the most efficient way to spend the Opioid
Settlement funds is through OMNI/EDGE counseling services. The amount will
allow OMNI to hire an LPC in-house at Buffalo Grove High School, 15 hours per
week for four years (2025-2028). The counseling services will provide youth,
families, and individuals with mental health and substance abuse support tailored to
the students' and family's needs. Services will include individual and group therapy,
mental health awareness education, after-hours family counseling, parent coaching
and mentoring/life skills.
A. Allocated funds will fully cover necessary services for insured and uninsured
individuals and/or families at a$75.00 per hour rate for 15 hours per week
annually for four years
i. OMNI will bill Medicaid
ii. EDGE will bill BCBS and Aetna
B. ONINUEdge will receive funding, no funds will be provided directly to
District 214
C. OMNI/EDGE will expend the funds in accordance with the opioid settlement
agreements.
D. If additional costs are needed, the funds will be paid by OMNI Youth
Services/EDGE Counseling Agency or District 214.
E. Based on estimated future funds the services will continue for four years.
2. Counseling sessions will take place at BGHS. If after hours/family sessions aze
warranted, the sessions will take place at the local OMNI or EDGE counseling
agency.
3. OMNUEDGE will be responsible for reporting monthly detailed performance
measures to the Village of Buffalo Grove.
