R-67-2 WHEREAS, the Vi 17 ego_ of aiiffaln Arrive located in the County of conk & Lake , State of Illinois, wishes to locate, construct, operate and maintain watermains, sewer lines. driveways, alleys, street returns. sidewalks, traffin signals and street lights constructed _either by the Vit t.g� of 1!nffsln arrays or under jupervion of said Village in the year of 1967 on the State Highways in _the Village of �'� y�aw comes under trio jurisdiction and control of the Department of Public {Works and Buildings of the State of Illinois. and 'WHEREAS, a permit from said Department is required before said work can be legally undertaken by said vi_136aget nr suparviiaart �y the Village now THEREFORE, be it resolved by the Village of Buffalo Grove. County of Cook & Lake , State of Illinois FIRST: That we do hereby request from the Department of Public Works and Buildings, State of Illinois, a permit authorizing the ,'Village of Buffalo Grqve ,to proceed with the work herein described. LORENZ, DIRECTOR VIRDEN E.STAFF DIVISION OF HIGHWAYS CHIEF HIGHWAY ENGINEER OFFICE OF THE DISTRICT ENGINEER ROOM 907 IN YOUR REPLY PLEASE 300 NORTH STATE STREET REFER TO FILE: CHICAGO 60610 December 15, 1966 YEARLY RESOLUTION FOR CONSTRUCTION ON STATE RIGHT OF WAY Dear Madam: State laws require that any person or municipality desiring to do any work on State Right of Way obtain a permit in writing from the Illinois Division of Highways. With each permit a bond is required to guarantee the State that the work shall be completed in accordance with conditions and restrictions contained in said permit. As a convenience to munitipalities, a resolution is acceptable in lieu of a bond. In order to expedite permits, this yearly resolution can be submitted by a Municipality guaranteeing the work authorized by all the permits procured during the calendar year. Attached hereto are resolution forms, as well as a sample of completed executed resolution. The resolution for the year of 1967, is to be adopted by the Board and signed by an authorized Municipal official and returned to this office for our files. On permits issued to a Municipality wherein the work is sub-contracted, a bond from the Contractor is required as well as a resolution from the Municipality. Both the Municipality and the Contractor assumes the responsibility of completing the work in accordance with the provisions contained in the permit.
