Approval of a Letter Agreement with USRLP Asbury Drive, LLC Regarding the Existing Declaration of Protective Covenants for the Property at 850 Asbury Drive (Trustee Weidenfeld) SUMMARY: As part of the recent approvals for 850 Asbury Drive, both staff and the developer seek approval of a letter agreement regarding the existing Declaration of Protective Covenants for the Asbury Drive Subdivision. The Village Attorney has read the letter and recommends approval. B. Gewalt Hamilton Associates, Inc 2017 Roadway Survey (Trustee Stein) SUMMARY: Staff seeks authorization to execute the attached agreement with Gewalt Hamilton Associates, Inc. for surveying services in an amount not to exceed $51,000.00. C. Approval of Local Public Agency Amendment# 1 for Federal Participation and Construction Engineering Services Agreement for Federal Participation -SUPPLEMENT #1 (Trustee Stein) SUMMARY: In 2012 the Village approved Resolution 2012-19 and 2012-22 for "Construction Engineering Services Agreement for Federal Participation"and Local Agency Agreement for Federal Participation for the Dundee Road Sidewalk Project. The agreements were between the Village of Buffalo Grove and Burns &McDonnell and the Village of Buffalo Grove and the State of Illinois (IDOT) respectively. IDOT approved $30,000 of additional work for Burns &McDonnell and the agreement needs to be modified to reflect the additional work. The Village is only responsible for$6,000, the other 80% is covered by Federal funding. This is the first supplement and amendment to these agreements. D. Award of Bid 2016 Sidewalk Project (Trustee Stein) SUMMARY: Staff recommends the Village Board award the bid for 2016 Sidewalk Project to the lowest responsive and responsible bidder, Suburban Concrete Inc. in an amount not to exceed $257,650.00. E. Award of Bid 2016 Sewer Lining - Phase 1 (Trustee Stein) SUMMARY: Staff recommends the Village Board award the bid for 2016 Sewer Lining- Phase 1 to the low bidder, Michels Pipe Services at an amount not to exceed $81,000.00. F. Award of Bid Arboretum Golf Club Carpeting Replacement (Trustee Johnson) SUMMARY: Staff recommends that the Village Board award the bid for the Arboretum Golf Club Carpeting Replacement project, to the lowest bidder Vortex Commercial Flooring Inc., in an amount not to exceed $57,681.60. G. Award of Bid Sidewalk Cutting (Trustee Stein) SUMMARY: Staff recommends the Village Board award the bid to the lowest responsive and responsible bidder Sidewalks Plus USA Ltd. In an amount not to exceed $58,580.00. H. Authorization to Execute Contract Extensions with Multiple Vendors (Trustee Stein) SUMMARY: Staff recommends that the Village President and Board of Trustees authorize staff to execute the following Contract extensions as listed below: A second year contract option with Aldridge Electric, for street light maintenance services. A second year contract option with Hillside Auto Body, for towing services. A third year contract option with Fire Safety Consultants, Inc. for third party plan review services. A second year contract option to Chicagoland Paving for asphalt patching. 0-2016-25 An Ordinance Amending Chapter 5.20 Liquor Controls (President Sussman) SUMMARY: The Class A liquor license for Auro Café, LLC d/b/a India House Restaurant at 228 McHenry Road is being changed to Auro Café, LLC d/b/a Mango Leaf Indian Restaurant and Banquet. The underlying ownership will remain the same. J. 0-2016-20 Ordinance Approving a Variation to the Sign Code for Culver's Restaurant Located at 450 McHenry Road (Trustee Weidenfeld) SUMMARY: Culvers is seeking to replace the three (3) existing wall signs on the building that require a variation to the Sign Ordinance. The Planning &Zoning Commission recommended unanimous approval of,,the variations. K. 0-2016-21 Ordinance Approving a Letter of Agreement Between the Village of Buffalo Grove and the Buffalo Grove Professional Firefighter/Paramedic Association (Trustee Trilling) SUMMARY: Staff recommends approval of a side letter agreement with the Buffalo Grove Professional Firefighter/Paramedic Association to include wages for the period of May 1, 2016 to April 30, 2017. L. 0-2016-22 Modifications to Chapter 13 of the Municipal Code (Trustee Stein) SUMMARY: The proposed ordinance enumerates the procedure for water meter replacement in the event that the meter's accuracy is called into question and testing is warranted. M. R-2016-11 Resolution for Authorization of Thompson & Brandywyn Phase I Design (Trustee Stein) SUMMARY: Staff seeks authorization to execute the attached agreement with Ciorba Group, for professional engineering design services for the Thompson and Brandywyn Phase I design. N. R-2016-12 Resolution Adopting a Revenue Collection Policy (Trustee Johnson) SUMMARY: The purpose of this document is to serve as a guide to identify major revenue sources, the method of collection, and the process of improving compliance rates. This policy was discussed with the Village Board during the Committee of the Whole Meeting on April 4, 2016. O. R-2016-13 Resolution Adopting Post-Issuance Procedures Manual for Tax-Exempt Bonds Issued by the Village of Buffalo Grove (Trustee Johnson) SUMMARY: Issuers of tax-exempt governmental Bonds must comply with federal tax rules pertaining to expenditure of proceeds for qualified costs, rate of expenditure, use of bond financed property, investment of proceeds in compliance with arbitrage rules, and retention of records. This policy addresses those requirements and was discussed with the Village Board during the Committee of the Whole Meeting on April 4, 2016. P. R-2016-14 Resolution Adopting the Village of Buffalo Grove Policy and Practice Relative to Debt (Trustee Johnson) SUMMARY: This policy will supersede the policy that was in effect from 2003. The major changes address the use of short-term debt, reaffirms the conditions that warrant the use of debt, and adds a glossary of terms. The policy was discussed with the Village Board during the Committee of the Whole Meeting on April 4, 2016. Q. R-2016-15 Resolution Adopting the Village of Buffalo Grove Investment Policy (Trustee Johnson) SUMMARY: The policy was discussed at the Committee of the Whole Meeting on April 4, 2016. The most significant change is a reduction is the collateral threshold amount for investments that exceed FDIC limits. This policy will supersede the former policy that was approved by Resolution 2008-33. 9. Ordinances and Resolutions A. 0-2016-23 Ordinance Permitting Video Gaming (Trustee Weidenfeld) B. 0-2016-24 An Ordinance Providing for the Issuance of$6,280,000*General Obligation Bonds, Series 2016, of the Village of Buffalo Grove, Lake and Cook Counties, Illinois, for the Purpose of Financing Street Improvements and Related Infrastructure Improvements, Providing for the Levy and Collection of a Direct Annual Tax Sufficient to Pay the Principal of and Interest on Said Bonds, and Authorizing the Sale of Said Bonds to the Purchaser Thereof(*Preliminary Amount, Subject to Change) (Trustee Johnson) C. R-2016-16 Resolution Approving an Intergovernmental Agreement with the Buffalo Grove Park District for Buffalo Grove Days (Trustee Stein) 10. Unfinished Business 11. New Business A. Questions From the Audience Questions from the audience are limited to items that are not on the regular agenda. In accordance with Section 2.02.070 of the Municipal Code, discussion on questions from the audience will be limited to 10 minutes and should be limited to concerns or comments regarding issues that are relevant to Village business. All members of the public addressing the Village Board shall maintain proper decorum and refrain from making disrespectful remarks or comments relating to individuals. Speakers shall use every attempt to not be repetitive of points that have been made by others. The Village Board may refer any matter of public comment to the Village Manager, Village staff or an appropriate agency for review. 13. Executive Session A.
