Proclamation Recognizing May as National Bike Month (Trustee Trilling) SUMMARY: The Village recognizes May as National Bike Month, including May 4th as Bike to School Day as well as May 16th to 20th as Bike to Work Week and May 20th as Bike to Work Day. B. Proclamation Recognizing National Emergency Medical Services Week (Trustee Trilling) SUMMARY: A proclamation recognizing May 15th through May 21st, 2016 as National Emergency Medical Services Week. C. National Public Works Week (Trustee Stein) SUMMARY:A proclamation recognizing May 15th through May 21st 2016 as National Public Works Week. D. A Proclamation Proclaiming May 15 to May 21 National Police Week (Trustee Trilling) v SUMMARY: A proclamation recognizing the week of May 15th to May 21st, 2016 as National Police Week. E. Approve Change Order Chadwick Contracting Company (Trustee Stein) SUMMARY: On April 4th, the Board authorized $83,737 in Metra Parking Lot Improvements to Chadwick Contracting Company. The Village previously received a $100,000 DCEO grant and has a cost sharing agreement with Pace for the shared access drive in an amount of$7,944.25. The change order amount will be covered by these two items at no cost to the Village. Staff recommends approval of a change order increase of up to$24,207 in additional patching quantities up to the amount of $107,944.25 as shown on the attached spreadsheet. F. Authorization to Execute an Agreement with Gewalt Hamilton Associates, Inc.for the 2017 Street Improvement Project Design (Trustee Stein) SUMMARY: Staff seeks authorization to execute the attached agreement with Gewalt Hamilton Associates, Inc. for design services in an amount not to exceed$46,700.00. G. Authorization to Purchase Road Salt for the 2016-2017 Winter Season (Trustee Stein) SUMMARY: Staff requests authorization to purchase not more than 1,440 tons of bulk rock salt from Morton Salt at$58.57 per ton at a total price not to exceed $84,341 and not more than 1,320 tons of bulk rock salt from Cargill Incorporated Salt Division at$71.90 per ton at a total price not to exceed$94,908 in accordance with the Illinois Governmental Joint Purchasing Act(30 ILCS 525/0.01 et seq.) H. 0-2016-26 Ordinance Concerning the Village of Buffalo Grove Administrative Hearing System (Trustee Berman) SUMMARY:The attached ordinance allows mobile telephone violations and other code violations to go through the adjudication process. 0-2016-27 An Ordinance Amending Chapter 5.20 Liquor Controls (President Sussman) SUMMARY: Hakuya Sushi, Inc at 161 W. Dundee Road has amended their liquor license renewal application from a Class B license to a Class E License. J. 0-2016-28 An Ordinance Amending Chapter 5.20 Liquor Controls (President Sussman) SUMMARY: The Class G7 license, authorizing the dispensing free of charge of beer and wine for consumption on the premises owned or leased and occupied by the licensee,will permit no more than 3 prearranged events per month at which beer and wine are offered for consumption on the premises in conjunction with an art gallery showing of art work, for a private function where attendance is by invitation only and not open to the public. Also expands categories for the G_license to include alcoholic liquor. K. 0-2016-29 An Ordinance Concerning Prevailing Wages (Trustee Berman) SUMMARY: The ordinance establishes the effective prevailing wages for both Cook and Lake counties for the year beginning June 1, 2016 and ending May 31, 2017. It should be noted that the attached prevailing wage rates(July 2015) are the most recent version published by the State of Illinois. Staff will update this information once it has been provided by the State. L. 0-2016-30 An Ordinance Amending Chapter 5.20 Liquor Controls (President Sussman) SUMMARY: The Class J liquor license for Roundy's Illinois, LLC d/b/a Mariano's Fresh Market at 450 Half Day Road is being changed to Roundy's Illinois, LLC d/b/a Mariano's at 450 Half Day Road. The underlying ownership will remain the same. 9. Questions From the Audience Questions from the audience are limited to items that are not on the regular agenda. In accordance with Section 2.02.070 of the Municipal Code, discussion on questions from the audience will be limited to 10 minutes and should be limited to concerns or comments regarding issues that are relevant to Village business. All members of the public addressing the Village Board shall maintain proper decorum and refrain from making disrespectful remarks or comments relating to individuals. Speakers shall use every attempt to not be repetitive of points that have been made by others. The Village Board may refer any matter of public comment to the Village Manager, Village staff or an appropriate agency for review. 13.
