WHEREAS, the Village of Buffalo Grove is a Home Rule Unit by virtue of the
Illinois Constitution of 1970; and,
WHEREAS, the real property legally described in EXHIBIT A ( "Property ") attached hereto'
is zoned in the R-4 One - Family Dwelling District with a special use for a public golf course and
related facilities; and,
WHEREAS, the Village of Buffalo Grove is the Owner of the Property; and,
WHEREAS, the Owner proposes to enter into a lease with T- Mobile Central LLC, a
Delaware limited liability company to construct a communications antenna and tower (120 feet in
height) and a ground -level equipment shelter for wireless communications pursuant to the following

telecommunications facility including a tower, antenna and equipment shelter pursuant to the
provisions and requirements of Chapter 17.34 of the Village Zoning Ordinance; and,

WHEREAS, notice of the public hearing conc'e'rning the petition for the special'use was'giGeri .

WHEREAS, `the Plan 'Cominission determined, pursuant to findings based on the testimony,,
presented at the public hearing, that the criteria for a Special Use as set forth in Section 1728,040 of
the Village's Zoning 'Ordinance have been met and`the special use is an appropriate use `'of the r
Property and will not unduly affect adjacent properties; "and,'
WHEREAS, the Corporate Authorities of the Village do hereby find that the proposed cellular
communications antenna and tower meet the special use criteria as set forth in Section 17.28.040 of
the Buffalo Grove Municipal Code and will comply with the provisions and requirements set forth in
Chapter 17.34 of the Village Zoning Ordinance, and specifically make the following findings and
conclusions regarding the proposed cellular communications antenna and tower
("telecommunications facility"):
1. It has been demonstrated that coverage for T— Mobile's wireless service in the area of the proposed'
telecommunications facility would be improved by construction of the proposed tower and antenna
thereby allowing greater cellular telephone use serving the public convenience not only at the
location of the installation but throughout the projected coverage area, and the establishment,
maintenance and operation of the telecommunications facility will not be detrimental to or endanger
the public health, safety, morals, comfort, or general welfare.
2. The nature and intensity of the proposed facility will be in harmony with the appropriate and
orderly development of the district in which it is located.

3. The distances to the nearest residences, the location of the site, and the open golf course area
adjacent to the site indicate that there will not be detrimental effects or injury to the use and
enjoyment of other property in the immediate vicinity of the subject property, nor will such use
substantially diminish and impair other property valuations within the neighborhood.
4. The nature, location and size of the telecommunications facility will not impede, substantially
hinder or discourage the development and use of adjacent land and buildings.
5. Utilities necessary to serve the telecommunications facility are adjacent to the site; there is
adequate. access to the site from the adjacent "existing parking lot and street; the proposed-
improvements. will not have any significant• impact upon drainage in the area; and any necessary,
facilities for the special use have beeti� or Will be •Provided. . , -.
6. The,telecommunications facility will require,maintenance visits only several times ,a year, and the.
facility can be served by existing parking, and .paved, access:. Said access is located so that there will
not be traffic congestion or hazards adjacent to the site or in'the public streets.

Section 2. The Corporate Authorities hereby approve an amendment to the Special Use
for the Property legally described in EXHIBIT A attached hereto.
Section 3. The special use is subject to the following:
A. The following variation of the Village Zoning Ordinance is hereby granted:
Section 17.40.020.B. — to allow a tower height of 120 feet instead of the
standard building height of 35 feet in the R-4 District;
B. The following variation of the Village Fence Code is hereby granted:
Section 15.20.040 — to allow fencing 8 feet in height to screen the ground —
level equipment shelter area, and the garbage dumpster area serving the
restaurant and clubhouse building.

C. The appearance and materials for the fences and gates depicted on Exhibit H
(site plan and elevations) hereto are subject to review and approval by the
Village.
D. A final landscaping plan shall be submitted to the Village Forester for review
and approval prior to removal of trees, site preparation work or grading on
the Property.

Petitioner
T- Mobile is a leading national wireless phone carrier and offers one of the finest wireless
communications networks in the nation. T- Mobile has entered into a lease agreement with the
Buffalo Grove Golf Course (owner) to install a wireless communications facility at the Course
located at 48 Raupp Boulevard.
Property Description
The Buffalo Grove Golf Course is located at 48 Raupp Road in the Village of Buffalo
Grove. The Golf Course is located on municipally owned property. T- Mobile proposes to install a
120' stealth flag pole with (6) antennas inside the pole and related equipment at grade level
adjacent to the pole. The facility will be located on a (13' -0" x 25' -0 ") lease area which will be
surrounded by a cedar fence. Access to the proposed facility will be via a new access easement
on the existing parking lot and driveway off of Raupp Boulevard. The proposed facility is located
in the R -4 Zoning District.
Nature of Request/Zoning Analysis
T- Mobile seeks a Special Use Permit and any other permits necessary to allow the
installation of its proposed wireless antenna facility.
Nature of Request /Zoning Analysis
T- Mobile seeks a Special Use permit (pursuant to the Table of Regulations below)
and any other permits necessary to allow the installation of its proposed wireless antenna
facility at the Buffalo Grove Golf Course.
Section 17.40.020 A. Table of Use Regulations (R -E through R -6A one family
dwelling districts) of the Buffalo Grove Zoning Ordinance states in relevant part:
Special Uses
Personal wireless communications facilities and cellular telephone facilities,
including antennas, for use with radio and /or other transmitting and receiving
equipment, on a tower or pole, on property owned by a unit of local government or
a public utility. Said tower or pole shall be set back from non - residential property
lines a distance not less than the height of the tower or pole. Said tower or pole shall
be setback from a residential structure and any abutting residential zoning
boundary a distance not less than twice the height of the tower or pole.
Components and Operations
The proposed facility will be unstaffed and, upon completion, will require only
infrequent maintenance visits (approximately one or two times a month) by a service
technician. Access to the proposed facility will be via a new access easement on the
parking lot and driveway off of Raupp Boulevard. Hence, the facility will not have any
material impact on traffic, parking or storm water control.
The proposed facility is entirely self - monitored by sophisticated computers which
connect directly to a central office and which alert personnel to equipment malfunction or
breach of security. Moreover, no material noise, glare, smoke, debris, traffic flow or any
other nuisance will be generated by the proposed facility.
The proposed facility will be designed and constructed to meet applicable
governmental and industry safety standards. Specifically, T- Mobile will comply with
FCC and FAA rules governing construction requirements, technical standards,
interference protection, power and height limitations and radio frequency standards.
Requested Action
T- Mobile respectfully requests a Special Use Permit and any other relief
necessary to accommodate the installation of a wireless antenna facility at the Buffalo
Grove Golf Course located at 48 Raupp Boulevard.
T- Mobile expressly reserves all of its rights, including those available to it under
the Buffalo Grove Code of Ordinances or any other state, local or federal law.

Special Use Criteria
A. All special uses shall meet the following criteria:
1. The special use will serve the public convenience at the location of the subject property;
or the establishment, maintenance or operation of the special use will not be detrimental to
or endanger the public health, safety, morals, comfort, or general welfare;
The special use will serve the public convenience at the location of the subject property and the
establishment, maintenance or operation of the special use will not be detrimental to or endanger
the public health, safety, morals, comfort, or general welfare; To the contrary, reliable wireless
technology provides vital communications that will be commonly used by local residents, businesses, and
emergency personnel for a wide variety of communication needs thereby promoting the general public's
health, safety, morals, comfort and overall general welfare. Utility networks such as electric, gas, water,
and wireless networks are essential to the development of any community. Wireless communications are
not a luxury anymore; they have become part of our lifestyle. Over 75 percent of all US households use a
wireless phone or data device and its projected that there will be more than 60 million new subscribers in
the next five years. Every day, wireless phone users place over 200,000 calls a day to 9 -1 -1 and other
emergency numbers to save lives and assist victims in accidents, crimes, fires, storms and natural
disasters. The increased use as resulted in a new caller location system called "Enhanced 911" (E911).
This system ensures that a wireless call is routed to the nearest emergency dispatch center. It also
provides emergency dispatchers with a call -back number of the distressed call and provides the
approximate location of the distressed caller. By improving the quality of the wireless infrastructure, the
proposed special use will provide improved wireless service to the Golf Club itself and hence serve the
public convenience at the golf club and beyond.
2. The location and size of the special use, the nature and intensity of the operation
involved in or conducted in connection with said special use, the size of the subject property
in relation to such special use, and the location of the site with respect to streets giving
access to it shall be such that it will be in harmony with the appropriate, orderly
development of the district in which it is located;
As depicted in the site plan, the proposed facility is diminutive in size in relation to the size of
the Buffalo Grove Golf Course. The lease area is (13' -0" x 25 -0 ") and will be enclosed by a
cedar fence. The lease area will be appropriately landscaped to blend in with the immediate
vicinity. Typically, once a site is built and operational, it is visited by a technician for routine
system checks once or twice monthly during normal business hours. There will be no traffic
congestion that will result from such visits. Access to the site will be via a new 20' wide access
easement on the existing parking lot and driveway off of Raupp Boulevard. For these reasons,
the location, size, nature and intensity of the special use will be in harmony with the appropriate,
orderly development of the district in which it is located.
3. The special use will not be injurious to the use and enjoyment of other property in the
immediate vicinity of the subject property for the purposes already permitted in such
zoning district, nor substantially diminish and impair other property valuations with the
neighborhood;
T- Mobile has been sensitive in selecting a site and design that will minimize the impact on the
surrounding property. The maintenance and operation of the proposed facility will be wholly
contained within a (13' -0' x 25' -0 ") lease area. Due to the diminutive size of the lease area and
the nature of wireless technology, there will be little impact on the use and enjoyment of the
property in the immediate vicinity for the purposes already permitted in the zoning district, nor
will there be any adverse effect on property values within the surrounding area. Improved
wireless communications will enhance the character of the neighborhood by improving public
safety, convenience and efficiency to many residents of the neighborhood.
4. The nature, location and size of the buildings or structures involved with the
establishment of the special use will not impede, substantially hinder or discourage the
development and use of adjacent land and buildings in accord with the zoning district
within which they lie;
T- Mobile has been sensitive in selecting and designing the proposed facility so that it does not
negatively impact the permitted uses or enjoyment of the properties in the immediate area or
impede, hinder or discourage development and use of adjacent land and buildings. The monopole
and related equipment will be enclosed by a cedar fence and will be appropriately landscaped to
blend in with the immediate vicinity. As stated above, reliable utility networks such as electric,
gas, water, and wireless networks are essential to the development of any community.
5. Adequate utilities, access roads, drainage, and/or other necessary facilities have been or
will be provided;
A wireless facility is unstaffed. The only utilities which will be required are telephone and
electricity, both of which are available on site. No drainage, sanitation, refuse removal, parks,
library, or school services will be necessary for this facility. This site is entirely self - monitored
and connects directly to a central office where sophisticated computers alert personnel to any
equipment malfunction or breach of security. Existing police and fire protection are more than
adequate to provide for this facility.
6. Parking areas shall be of adequate size for the particular special use, which areas shall
be properly located and suitably screened from adjoining residential uses, and the entrance
and exit driveways to and from these parking areas shall be designed so as to prevent
traffic hazards, eliminate nuisance and minimize traffic congestion in the public streets.
A wireless facility is unstaffed and will be visited once or twice monthly by maintenance
personnel, and thus, there will be no need for any additional parking spaces in the existing,
adjacent lot to accommodate these visits. With just a couple visits each month, there will be no
traffic hazards nor any other nuisances to traffic patterns nor any adverse affect on traffic
congestion in the public streets. Access to the site will be via a new 20' wide access easement on
the existing parking lot and driveway off of Raupp Boulevard.
B. The special use shall in all other respects conform to the applicable regulations of the
zoning district in which it is located, except as such regulations may be varied.

To Whom It May Concern:
T- Mobile has classified different levels of signal strength to adequately serve varying
grades of users. In short T- mobile has categorized signal level into 3 different categories
to enable it to design its network and thus provide the desired coverage with a high
degree of confidence. The different signal levels correspond to different colors on a
propagation map and are defined as follows:
Green: -76 dBm < = Signal Level. Level required for In- Building coverage
Yellow: -84dBm < = Signal Level < - 76d.Bm. Level required for In- Vehicle coverage
Light Grey: -91 dBm < = Signal Level < - 84dBm. Level required for Outdoor coverage
T- Mobile's current coverage in the village of Buffalo Grove based on the aforementioned
thresholds and propagation maps supplied to the village can be qualified as reliable In-
Vehicle coverage. The purpose of this site is to improve the gaps in residential coverage
that exist along Lake Cook Rd. between Buffalo Grove Rd and Arlington Height Rd as
well in the residential area South of Lake Cook Rd. The proposed communication tower
at 50 Raupp Blvd. in Buffalo Grove will greatly help in achieving the coverage objective.

Dear Mr. Howley:
Based on the information you provided, T- Mobile intends to operate their equipment at the site located at
48 Raupp Boulevard, Buffalo Grove, IL 60089 on the basis of FCC licenses which exclusively entitles the
company to operate within an assigned frequency range of 1970 -1990 and 2110 -2120 MHz. Their
operating frequencies preclude the potential of interference issues with Village of Buffalo Grove public
safety services located at a tower in proximity of the proposed location. I have received and reviewed the
city's Antenna Coax Color Code Chan` regarding what is on the village hall tower. Based upon the
information provided, I conclude that there will be no interference.
In the unlikely event that interference is created, T- Mobile, as a condition of their FCC licenses, assumes
full responsibility to take necessary measures to resolve these issues.

Commission ( "the FCC" and "the FCC Rules ") both in general and specifically as they apply to
the FCC's Guidelines for Human Exposure to Radiofrequency Electromagnetic Fields; and
That the technical information serving as the basis for this report was supplied by T- Mobile (See
attached Site Summary and Carrier documents), and that T- Mobile's installations involve
communications equipment, antennas and associated technical equipment at a location referred to
as the "CH33 -259H - Buffalo Grove Golf Course" ("the site "); and
That T- Mobile proposes to operate at the site with transmit antennas listed in the carrier summary
and with a maximum effective radiated power as specified by T- Mobile and shown on the
worksheet, and that worst -case 100% duty cycle have been assumed; and
That this analysis has been performed with the assumption that the ground immediately
surrounding the tower is primarily flat or falling; and
That at this time, the FCC requires that certain licensees address specific levels of radio -
frequency energy to which workers or members of the public might possibly be exposed (at
§ 1.1307(b) of the FCC Rules); and
That such consideration of possible exposure of humans to radio- frequency radiation must utilize
the standards set by the FCC, which is the Federal Agency having jurisdiction over
communications facilities; and
That the FCC rules define two tiers of permissible exposure guidelines: 1) "uncontrolled
environments," defined as situations in which persons may not be aware of (the "general
public "), or may not be able to control their exposure to a transmission facility; and (2)
"controlled environments," which defines situations in which persons are aware of their potential
for exposure (industry personnel); and
That this statement specifically addresses the uncontrolled environment (which is more
conservative than the controlled environment) and the limit set forth in the FCC rules for
licensees of T- Mobile's operating frequency as shown on the attached antenna worksheet; and
That when applying the uncontrolled environment standards, the predicted Maximum Power
Density at two meters above ground level from the proposed T- Mobile operation is no more than
0.236% of the maximum in any accessible area on the ground and

That it is understood per FCC Guidelines and OET65 Appendix A, that regardless of the existent
radio - frequency environment, only those licenses whose contributions exceed five percent of the
exposure limit pertinent to their operation(s) bear any responsibility for bringing any non-
compliant area(s) into compliance; and
That the calculations provided in this report are based on data provided by the client and antenna
pattern data supplied by the antenna manufacturer, in accordance with FCC guidelines listed in
OET -65. Horizontal and vertical antenna patterns are combined for modeling purposes to
accurately reflect the energy two meters above ground level where on -axis energy refers to
maximum energy two meters above the ground along the azimuth of the antenna and where area
energy refers to the maximum energy anywhere two meters above the ground regardless of the
antenna azimuth, accounting for cumulative energy from multiple antennas for the carrier and
frequency range indicated; and
That the Occupational Safety and Health Administration has policies in place which address
worker safety in and around communications sites, thus individual companies will be responsible
for their employees' training regarding Radio Frequency Safety.
In summary, it is stated here that the proposed operation at the site would not result in exposure
of the Public to excessive levels of radio - frequency energy as defined in the FCC Rules and
Regulations, specifically 47 CFR 1.1307 and that T- Mobile's proposed operation is completely
compliant.
Finally, it is stated that access to the tower should be restricted to communication industry
professionals, and approved contractor personnel trained in radio- frequency safety; and that the
instant analysis addresses exposure levels at two meters above ground level and does not address
exposure levels on the tower, or in the immediate proximity of the antennas.

Proposed telecommunications facility — antenna and tower
WHEREAS, the Village of Buffalo Grove is a Home Rule Unit by virtue of the
Illinois Constitution of 1970; and, '
WHEREAS, the real property legally described in EXHIBIT A ( "Property") attached hereto''
is zoned in the R-4 One - Family Dwelling District with a special use for a public golf course and
related facilities; and,
WHEREAS, the Village of Buffalo Grove is the Owner of the Property; and,
W "EREAS, the Owner proposes to enter into a lease with T- Mobile Central ?LC, a
Delaware limited liability company to construct a communications antenna and tower (120 feet in
height) and a ground -level equipment shelter for wireless communications pursuant to the following

WHEREAS, Insite Real Estate Inc., as agent for T— Mobile Central LLC, a Delaware limited.`
liability company, has submitted a petition requesting approval of a special use for a .
telecommunications facility including a tower, antenna` and equipment shelter pursuant to the
provisions and requirements of Chapter 17.34 of the Village Zoning Ordinance; and, ' -J
WHEREAS, notice of the public hearing concerning the petition for the special use was'given'
and the public hearing was held by the Buffalo Grove Plan Commission on April 7, 2010; - and; ~.,'
WHEREAS, 'the Plan Commission determined, pursuant to findings based on the testimony,
presented at the public hearing, that the criteria for a Special Use as set forth in Section 17.28.040 o -
the Vill 'Z
age's oning 'Ordinance have keen met and the special use is an appropriate use -of the
Property and VMI not unduly affect adjacent properties; 'and,
WHEREAS, the Corporate Authorities of the Village do hereby find that the proposed cellular
communications antenna and tower meet the special use criteria as set forth in Section 17.28.040 of
the Buffalo Grove Municipal Code and will comply with the provisions and requirements set forth in
Chapter 17.34 of the Village Zoning Ordinance, and specifically make the following findings and
conclusions regarding the proposed cellular communications antenna and tower
( "telecommunications facility "):
1. It has been demonstrated that coverage for T— Mobile's wireless service in the area of the proposed
telecommunications facility would be improved by construction of the proposed tower and antenna
thereby allowing greater cellular telephone use serving the public convenience not only at the
location of the installation but throughout the projected coverage area, and the establishment,
maintenance and operation of the telecommunications facility will not be detrimental to or endanger
the public health, safety, morals, comfort, or general welfare.
2. The nature and intensity of the proposed facility will be in harmony with the appropriate and
orderly development of the district in which it is located.

3. The distances to the nearest residences, the location of the site, and the open golf course area
adjacent to the site indicate that there will not be detrimental effects or injury to the use and
enjoyment of other property in the immediate vicinity of the subject property, nor will such use
substantially diminish and impair other property valuations within the neighborhood.
4. The nature, location and size of the telecommunications facility will not impede, substantially
hinder or discourage the development and use of adjacent land and buildings.
5. Utilities necessary to serve the telecommunications facility are adjacent to the site; there is
adequate access to the site from the adjacent existing parking lot and street; the proposed
improvements. will; not have any significant: impact upon drainage in the area; and any necessary,
facilities for the special use have been or,Will be provided .' .
6. The telecommunications facility will require maigtenance visits only several times a year, and the
facility, can. be served by existing parking,and paved acces$.. Said access is located so that there will
not be traffic congestion or hazards adjacent to the site or in'the public streets.
7. The special use in all other respects conforms to the applicable regulations of the R-4 zoning
district ? except as such regulations are varied in this, Ordinance
NOVA, THEREFORE, BE IT ORDAINED` BY THE PRESIDENT AND BOARD OF '
TRUSTEES OF THE VILLAGE OF BUFFALO GROVE, COOK AND LAKE COUNTIES, r
I LINOIS,
Section 'l . _ This ordinance is made pursuant to and in accordance with the Village's
Zoning Ordinance and the Village's Home Rule powers. The preceding whereas clauses are hereby
made a part of this Ordinance.
Section 2. The Corporate Authorities hereby approve an amendment to the Special Use
for the Property legally described in EXHIBIT A attached hereto.
Section 3. The special use is subject to the following:
A. The following variation of the Village Zoning Ordinance is hereby granted:
Section 17.40.020.B. — to allow a tower height of 120 feet instead of the
standard building height of 35 feet in the R-4 District;
B. The following variation of the Village Fence Code is hereby granted:
Section 15.20.040 — to allow fencing 8 feet in height to screen the ground —
level equipment shelter area, and the garbage dumpster area serving the
restaurant and clubhouse building.

C. The appearance and materials for the fences and gates depicted on Exhibit H
(site plan and elevations) hereto are subject to review and approval by the
Village.
D. A final landscaping plan shall be submitted to the Village Forester for review
and approval prior to removal of trees, site preparation work or grading on
the Property.
E. This special use is contingent upon the Village of Buffalo Grove and T—
Mobile . Central LLC entering into a lease concerning the proposed
telecommunications facility. 'In ithe event that a lease is not executed, the
Village may, in its. sole :discretion void ` the special use granted by this
Ordinance.
Section 4: This Ordinance: shall be' in full force and effect on, and after its passage and
approval. This Ordinance shall not be- codified:
